This trial is for adults over 18 who are part of the Mobile Health and Wellness Program, speak English, and have hypertension, diabetes, heart failure or COPD. They must be able to use a remote monitoring device or phone for care and take medication for blood pressure/diabetes.Inclusion Criteria
