Telehealth Follow-up for PICS

This trial explores whether telehealth follow-up appointments with a pharmacist can improve recovery for individuals who have survived a critical illness and been discharged from the intensive care unit (ICU). The goal is to determine if this approach, known as Post-ICU Telehealth Follow-up, can effectively support recovery from post-intensive care syndrome (PICS), which includes issues like weakness, cognitive difficulties, and emotional distress after leaving the ICU. Participants will receive either the telehealth follow-ups or the usual post-ICU care. The study seeks individuals who have spent a significant time in the ICU and are preparing to return home after their hospital stay.
